Dr. Jeff
Family Physician
Dr. Jeff Panzer is a family physician and has been practicing in Chicago since 2010. He attended Temple University School of Medicine and did his family medicine residency at Thomas Jefferson University Hospital in Philadelphia, PA. He later received his Master’s degree in healthcare quality and patient safety from Northwestern University. Since graduating residency in 2009, Dr. Jeff has worked both caring for patients and at the system level to try to bring relationships back to the forefront of healthcare where they rightfully belong.  He previously served as the vice president of care transformation for Tapestry 360 Health (a federally-qualified health center on the northside of Chicago) and as the physician innovator at AllianceChicago. He also previously served as a medical director at a health center in rural Ethiopia, and as a practice transformation consultant at the American Medical Association (AMA). He can provide patient care in English and Spanish. Outside of the office, Dr. Jeff enjoys spending time with his wife and three children, running, beach volleyball, reading, camping, and coaching his children’s sports teams.

Dr. Emma
Family Physician
Dr. Emma Daisy is a Board Certified Family Physician with added certification in Geriatrics. She graduated from Northwestern’s own Feinberg School of Medicine in 2011 and completed her family medicine residency at Northwestern/Erie Family Health in 2014, where she first met Dr Jeff. She completed a fellowship in geriatrics at the University of Chicago in 2015, and has been practicing in the Chicago area since then. In addition, she has served in various capacities for the Illinois Academy of Family Physicians, including as president and as board chair. Her interests lie in health equity and primary care access, and she remains active in the Illinois and American Academies of Family Medicine, which give her a platform to promote better access to high quality primary care for all. In her own practice, she views herself as a family physician first and foremost, and loves taking care of patients of all ages and life stages. When not at work, Dr Daisy can be found doing yoga, biking, or swimming in Lake Michigan. She enjoys hiking and camping, and makes regular visits to the mountains of North Carolina or Virginia, where she grew up. She is the mother of two and married to a professional musician, so her days are filled with both music and a joyful noise.

Guadalupe
Patient Care Coordinator
Lupe is the Patient Care Coordinator at Bridge Family Health, where she plays a vital role in supporting patients throughout their health care journey. Her work includes rooming patients, assisting with visits, and coordinating tests, labs, and procedures. Lupe is dedicated to ensuring every patient has a smooth, comfortable, and seamless experience. She is a bilingual Certified Clinical Medical Assistant and Certified Phlebotomist, bringing a multifaceted skill set and wealth of experience to the office. Outside of work, she enjoys spending quality time with her family.

Gulianna
Outreach & Engagement Coordinator
Gulianna is the Patient Outreach and Engagement Coordinator at Bridge Family Health. She manages communications and social media, builds community partnerships, and leads outreach to help more people discover Direct Primary Care. She earned her degree from Marquette University, majoring in Digital Media with a double minor in Public Relations and Advertising. With a background in digital media and communication, Gulianna brings clarity, creativity, and connection to her role. Outside of work, she enjoys long walks with her sisters, exploring local spots, and discovering new shows to watch.

Ana
Office Manager
Ana is the Office Manager and Coordinator at Bridge Family Health. She supports patients by guiding them through onboarding and ensuring they receive support throughout their healthcare journey. A Certified Community Health Worker and Certified Care Coordinator, Ana brings both expertise and compassion to her role. Currently pursuing her nursing degree at Chamberlain University, she is passionate about primary care and community health. Ana finds fulfillment in helping patients navigate the healthcare system and connecting them with the benefits and resources available to them. Outside of work, she enjoys spending time with her son and their cat, watching and playing basketball, and relaxing with a good movie.
