Family medicine built around relationships

Primary care works better when your doctor actually knows you.

At Bridge, you have an ongoing relationship with a family physician who has time to understand your health history, concerns, goals, medications, family history, and what matters to you.

Our direct primary care model allows us to offer longer appointments, easier access to care, and direct communication with your care team without billing insurance for the primary care services included in your membership.

The goal is simple: healthcare that feels personal again.

Meet your Evanston family physicians

Choosing a primary care physician is one of the most personal healthcare decisions you'll make. We're honored to be part of that journey.

Dr. Jeff Panzer and Dr. Emma Daisy believe the best care begins with trust, communication, and lasting relationships. They take the time to listen, answer questions, and partner with patients through every stage of life.

Dr. Jeff Panzer

Dr. Emma Daisy
