Care Starts With People Who Know You

At Bridge Family Health, great primary care is built on relationships. Our physicians and care team take the time to know you, understand what matters to you, and make healthcare feel a little more personal.

Healthcare can feel complicated. Your relationship with your primary care team shouldn't.

At Bridge, you will see familiar faces, work with physicians who know your history, and have a team you can turn to when questions come up. From scheduling an appointment to managing an ongoing health concern, we want every interaction to feel thoughtful, welcoming, and personal.

That commitment extends beyond our physicians. Everyone on the Bridge team plays a role in creating an experience where patients feel heard, respected, and cared for.

Patient Care Coordinator & Medical Assistant | Certified Medical Assistant & Phlebotomist

Languages Spoken: English & Spanish

As Bridge’s Patient Care Coordinator and Medical Assistant, Lupe supports patients throughout their experience at Bridge. From intake, vitals, and lab collection to coordinating appointments, answering questions, and helping patients understand their next steps, she helps make care feel easier to navigate.

Lupe was drawn to healthcare by her love of helping people and making a positive difference in their day. She especially values having the time at Bridge to listen, get to know patients, and build meaningful relationships with them. Her goal is for every patient to feel welcomed, comfortable, heard, and cared for.

Outside of Bridge, Lupe enjoys spending time with her family, going for walks, and being outdoors. And a fun fact: given the choice between cooking and cleaning, she will happily choose cleaning.

Office Manager/Coordinator | Certified Community Health Worker

Languages Spoken: English & Basic Spanish

Ana has been part of Bridge Family Health from the beginning, helping Dr. Jeff build the practice from the ground up. As Bridge’s Office Manager/Coordinator, she is a self-described “jack of all trades” and problem solver who helps keep the practice running smoothly while supporting patients with everything from onboarding and questions to navigating the healthcare system.

What Ana loves most about Bridge is the personal approach to care. She wants every patient who calls or walks through the door to feel comfortable, welcomed, and confident asking for help. Even if she does not have the answer herself, she will work to find it or connect patients with the right person.

Before Bridge, Ana worked as a Patient Access Specialist in the emergency room and later alongside Dr. Jeff at Tapestry. Today, she is continuing her education in nursing school, inspired in part by her years of working with patients. Outside the office, she enjoys spending time with her son, traveling, and trying new things.

A Longstanding Partnership

Dr. Emma and Dr. Jeff have known each other professionally for more than 15 years, with their paths crossing throughout their careers in family medicine. Before practicing together at Bridge, they worked within the same healthcare organization from 2018 to 2025. That longstanding professional relationship gives them a shared foundation in family medicine and a deep understanding of how each other approaches patient care.

Jimmy C, Patient

"Working with Dr. Jeff is truly a collaboration, he listens, and respect the patient. He does not rush you in your appointment and you work together for what works for you. Lupe is wonderful with the administration and doing the blood work."
